When the U. S. Food and Drug Administration (FDA) announced that the first-line treatment for erectile dysfunction (ED) would be marketed as Viagra (sildenafil citrate), they made it clear they wanted to give consumers a new option that would work in the same way that Viagra had before it was approved.
But when the FDA decided in 2013 to approve the drug in a lower dosage and lower price, it also made it clear that they wanted its side effects and the safety of the drug in place.
In a statement to the New York Times, the agency said, “The FDA’s decision to approve the drug for use in the United States reflects its commitment to advancing the health, safety and well-being of Americans.”
But a warning letter to the agency did not mention that the drug may cause erectile dysfunction. The FDA did not specify how many people would be affected.
In the letter, FDA Chairman Scott Gottlieb said the agency “strongly believes that the public is informed about this potential safety risk and will take appropriate steps to minimize the risk and ensure appropriate treatment.”
He also said the agency wanted to emphasize that it is committed to patient safety, not the safety of drugs. The letter is an attempt to get people to take responsibility for their own medical decisions. The agency also wrote to the patients’ representatives that it is “the FDA’s highest priority to provide information to health care providers to help them make an informed decision about their health.”
In an interview with the Times, Gottlieb acknowledged the FDA’s decision to approve Viagra because it was “very concerned that the information provided would not be as helpful to patients.”
“I would never think that this would happen again,” he said. “We’re making a decision based on the information available to us and not on our patients’ concerns.”
The FDA will continue to work with the manufacturer of Viagra and other ED drugs to make sure Viagra is safe and effective for the millions of people who take it each day.

Enzalutamide
Use alternative drugs because the use of enzalutamide will decrease the effect of Sildenafil by affecting patient hepatic and intestinal enzyme metabolism.
Acetazolamide
Therapy should be administered with caution because the use of Sildenafil increases the effect of acetazolamide by pharmacodynamic synergism and it also increases the risk of hypotension.
The use of this medicine can lower blood pressure and when combining it with alcohol further increases the effect. You may experience dizziness, lightheadedness, fainting, flushing, headache, and heart palpitation. Also, avoid consuming grapefruit juice because it may increase the effect of avanafil on blood levels.
Cardiovascular disease:
The use of phosphodiesterase-5 enzyme inhibitors is contraindicated in patients with cardiovascular disease. A doctor should consider the vasodilatory effects of this drug and whether they may affect patients with cardiovascular disease. The use of this drug may increase the risk of angina pectoris, AV block, myocardial infarction, ventricular arrhythmia, tachycardia, palpitation, hypotension, postural hypotension, syncope, cerebral thrombosis, cerebrovascular hemorrhage, transient ischemic attack, cardiac arrest, heart failure, and hypertension. These events can occur during or after sexual activity.
Renal dysfunction:
Therapy should be administered with caution in patients with severe renal disease or on renal dialysis. The plasma clearance of this drug is decreased in patients with a severe renal impairment which results in drug accumulation.
Liver diseases:
The use of this drug is not recommended for patients with severe hepatic impairment because the pharmacokinetic disposition of this drug has not been assessed in patients with severe hepatic impairment.

Heart arrhythmias:

Therapy should be administered with caution in patients with supine or antidepression and in patients with controlled hypotension.
